  • Positions will cover areas surrounding Houston, TX.
  • With this innovative role comes the opportunity to contribute directly to the success and development of an exciting new program.
  • The On-site Case Manager works to coordinate the transitions between hospitals, rehab centers and long-term care facilities and patient homes, ensuring that appropriate care is provided during all stages.
  • You would also provide care management to enrollees admitted to post acute care facilities and occasionally while the patient is hospitalized.
Primary responsibilities include:
  • Rounding at post acute care facilities to meet with the patient/member
  • Working with the physician, nurse and/or discharge planner and participating in the patient's transition to an acute rehab or skilled nursing facility
  • Monitoring the patient throughout the transition and conducting concurrent review, as well as on-site and case management
  • Coordinating authorizations for services
  • Providing updates and following up on patient status
  • Participating in team meetings at the facility Building strong relationships with the facilities and staff

Qualifications
Qualifications:
  • Current and unrestricted RN license for the state of Texas required
  • 3+ years clinical experience in acute/rehab/or long term care setting
  • Comprehensive knowledge of Medicare/Medicaid regulations
  • Proficient computer skills
  • Must be a self-starter who demonstrates strong interpersonal skills and excellent communication skills.
  • Ability to travel frequently in assigned territory. Reliable transportation.
  • Acess to high speed internet service in location you would be using as home office (Cable modem, DSL, Wireless, etc..)
  • Discharge planning experience
Additional preferences include:
  • CCM certification
  • Case Management experience
  • Knowledge or experience with Milliman or Interqual guidelines
  • Bilingual English/Spanish
