  • UT Health East Texas Rehabilitation Center includes a 48-bed inpatient hospital, 14 outpatient therapy clinics, 11 cardiopulmonary rehabilitation clinics and nine Olympic fitness centers, with access to some of the most advanced therapy techniques and care available.

Responsibilities

  • Evaluates and assesses the client based on subjective and objective information obtained through medical record review, formal and informal test administration with appropriate analysis and interpretation of results to develop the plan of care. Includes OASIS assessments at appropriate time points.
  • Formulates written functional measurable goals, treatment plans, and home programs for clients. Identifies barriers to desired outcomes. Evaluates effectiveness and outcomes of care. Plans for discharge.
  • Provides treatment according to established goals on the treatment plan and reassesses the client's functional status when goals are met or there is a change in the client's condition. Informs physician and other personnel of changes in the client's needs. Revises plan of care as appropriate.
  • Instructs/educates clients/caregivers in appropriate activities and referral to community resources as appropriate utilizing both verbal and written programs.
  • Documents evaluations and progress in the clinical record and maintains client records as required by the Director.
  • Communicates with clients, caregivers, and all health care professionals required to provide comprehensive interdisciplinary care.
  • Attends Case Conferences when speech/language therapy clients are discussed.
  • Maintains skills and knowledge through training and continuing education to provide expected standards of care.

Qualifications

Job Requirements:

  • Master's Degree in Speech Language Pathology.
  • Completion of clinical fellowship year (CFY).
  • Current professional licensure in the State of Texas.
  • Certificate of Clinical Competence by the American Speech/Language, Hearing Association.
  • Must have a current Texas driver's license.
