Summary:

The Family Advocatewill work as part of Cook Children's Physician Network in our neighborhood clinics to providesupport and resources to families for issues that extend beyond their clinical needs. Duties include:

  • Build individual and family capacity by increasing health knowledge and self-sufficiency.

  • Responds to the social and emotional needs of our families, providing guidance and support while navigating them through the health care system and community to achieve the best health outcomes.

  • Serve as both an advocate and liaison between the patient/family, health care team, and community partners.

  • Functions in collaboration with the treatment team within the Cook Children's Neighborhood Clinics.

Hours: Mon-Fri 8am-5pm

***This position will provide coverage to 2 clinics. All neighborhood clinics are located in Tarrant County ***

Required Experience & Education:

  • 3 years' experience working with vulnerable populations (or) working in healthcare.

  • High School diploma or equivalent.

  • Strong customer service, phone, and computer skills

  • Familiarity with basic medical terminology.

  • Strong organizational skills with the ability to prioritize.

  • Demonstrate problem-solving skills.

  • Ability to work autonomously with minimal supervision.

Preferred Education, Experience & Skills:

  • Bachelor's degree in Social Work.

  • Experience working with families and social services.

  • Experience providing psychosocial assessments & crisis intervention.

  • Bilingual: Spanish & English speaking.
