Responsibilities

  • Works as an active member of the multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide robust individualized patient-centered care
  • Leads assigned patient service lines, clinical areas, and related therapeutic programs
  • Delivers clinical pharmacy practice, including decentralized and service-based programs
  • Participates in daily multidisciplinary and bedside rounds when scheduled
  • Facilitates pharmaceutical care services and medication utilization systems within assigned services and care areas to ensure drug utilization activities are aligned with patient care needs, evidence-based best practices, and regulatory standards
  • Educates and directs pharmacy personnel by actively participating in staff meetings and attending staff development in pharmaceutical care to identify learning and performance needs.
  • Makes appropriate evidence-based, patient-centered medication recommendations
  • Must serve as a preceptor for pharmacy students and/or residents.
  • Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose
  • Participates in the management of medical emergencies as appropriate, including by not limited to Code Blue response
  • Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed
  • Supports and leads in the implementation of initiatives supporting the goals of the organization's quality improvement, risk management and patient safety program including but not limited to National Patient Safety Goals, Core Measures and HCAHPS medication-related initiatives
  • Sustains the hospital formulary, minimizing non-formulary procurements, utilizing therapeutic interchanges and other approved formulary protocols and/or guardrails.
  • Effectively and proactively interacts with medical staff regarding drug therapy issue resolutions
  • Participates in pharmacy operations and medication dispensing as needed
  • Maintains proficiency in and actively engages in operational programs, central dispensing pharmacies, and satellite dispensing pharmacies as appropriate for job assignment
  • Maintains appropriate controlled substance records

Qualifications

Job Requirements

  • B.S. or Pharm.D. from an ACPE-accredited School of Pharmacy
  • Licensed or eligible for licensure in Texas
  • Clinical pharmacy experience in a hospital is defined as the following:
    • Completion of an accredited PGY-1 Pharmacy residency or fellowship or 2 years of clinical pharmacy experience with no history of residency
  • ACLS and BLS certification or completion within 90 days of hire

Preferred Job Requirements:

  • ACLS and BLS Certified
  • IV Certification
  • Board of Pharmacy Specialties Certification
  • Texas Preceptor Certification
